Description

Mobility support can be arranged if you are unable to use public transport independently due to an illness, injury or similar reason that reduces your ability to function and you need support in running errands or other everyday needs. You can be assigned a support person for getting around outside your home based on a service needs assessment. With a support person, you can practice using public transport independently, improve your ability to get around independently and use a travel card. Mobility support can be provided in the following ways or a combination of them: - guidance and guided training in the use of public transport - escort service - group transportation - by reimbursing the reasonable costs of transport by taxi, disabled taxi or other similar vehicle - by other appropriate means Support person activities You can be assigned a volunteer support person to help you get around outside your home based on a service needs assessment. The support person can help you practice using public transport and travel cards independently and strengthen your ability to get around independently outside your home. Transport services You are eligible for the transport service if two conditions are met. Firstly, you have had significant mobility difficulties for your age for at least a year. Secondly, you are unable to use public transport without great or unreasonable difficulty despite support measures (e.g. escort service). The need for the transport service is assessed by a professional in a service needs assessment.

Conditions

You are eligible for the mobility support services if you are unable to use public transport independently without unreasonable difficulty due to an illness, injury or similar reason that reduces your ability to function. To receive the service, please contact the counselling service for people with disabilities either by phone, email, through the Maisa client portal or by sending an application online or in paper form. Neuvolaikäisten psykologit ja kuraattorit tukevat lapsen oppimista, hyvinvointia sekä kasvua ja kehitystä. Neuvolaikäisten psykologit ja kuraattorit toimivat tiiviissä yhteistyössä huoltajien, neuvolan, lasten kuntoutuspalvelujen sekä varhaiskasvatuksen ja esiopetuksen kanssa. Psykologipalvelut kuuluvat kaikille neuvolaikäisille lapsille, mutta kuraattorin palveluita saavat vain esiopetusikäiset lapset. Esiopetuksen psykologi- ja kuraattoripalvelut ovat osa opiskeluhuollon palveluita. Esiopetuksessa psykologien ja kuraattorien työhön kuuluu yksilöllisen työn lisäksi yhteisöllinen työ, jossa tuetaan koko esiopetusyksikön hyvinvointia. Lapsen asiakkuus kirjataan asiakas- ja potilastietojärjestelmä Apottiin.

How to use it

Vanhempana voit ottaa yhteyttä kuraattoriin, jos sinulla on huoli lapsestasi tai perheesi tilanteesta. Voit varata ajan puhelimitse tai sähköpostitse. Asiakkaaksi voi ohjautua myös päiväkodin työntekijän yhteydenoton kautta. Voit saada lapsellesi lähetteen psykologille neuvolasta tai lasten kuntoutuspalveluista esimerkiksi puheterapiasta tai toimintaterapeutilta. Varhaiskasvatuksesta voidaan konsultoida psykologia huoltajan luvalla. Esiopetusikäisen lapsen asioissa voit lapsen huoltajana ottaa psykologiin yhteyttä myös itse. Esiopetuksen psykologipalvelut ovat osa opiskeluhuollon palveluita ja työtä tehdään tiiviissä yhteistyössä esiopetuksen kuraattorin kanssa. Perhevalmennus tarjoaa tukea lasta odottaville perheille valmistautumiseen vanhemmuuteen ja vauva-arkeen. Valmennuksessa käsitellään muun muassa synnytystä, vauvan hoitoa ja perheen hyvinvointia. Palvelu on maksuton ja tarkoitettu kaikille odottaville perheille Vantaan ja Keravan alueella. Neuvolan perhevalmennusta tarjotaan hyvinvointialueemme ensimmäistä lastaan odottaville perheille. Myös muut lasta odottavat perheet ovat halutessaan tervetulleita. Perhevalmennuksen tavoitteena on antaa tietoa raskauteen, synnytykseen ja vauvanhoitoon liittyvissä asioissa sekä tukea vanhemmuuteen kasvamisessa ja vahvistaa vanhempien ja vauvan välistä vuorovaikutussuhdetta. Perhevalmennus koostuu kahdesta tapaamisesta. Valmennuksesta keskustellaan raskauden ensikäynnillä ja aiheeseen palataan uudelleen raskausviikon 22 käynnillä, jolloin ajanvaraus on ajankohtainen. Ensimmäinen, synnytystä ja vauvan ensimmäisiä päiviä käsittelevä etäsynnytysvalmennus toteutetaan yhteistyössä synnytyssairaalan kanssa. Etäsynnytysvalmennukseen osallistumisen ohjeet löytyvät Vauvamatkalla -digipolulta. HUS aktivoi Vauvamatkalla-digihoitopolun jokaiselle odottajalle raskauden ensimmäisen ultraäänitutkimuksen yhteydessä tai noin raskausviikolla 12. Linkki ja valmennusajankohdat löytyvät Vauvamatkalla-digihoitopolulta kohdasta "Valmistautuminen vauvan syntymään ja imetykseen, synnytysvalmennus". Valmennuksessa käydään läpi synnytyksen vaiheita, kivunlievitystä ja vauvan ensimmäisiä päiviä. Valmentajina toimivat HUSin kätilö ja neuvolan terveydenhoitaja Helsingistä, Espoosta, Vantaalta tai Keravalta. Toinen valmennuskerta järjestetään hyvinvointilalueemme neuvoloissa joko läsnä- tai etävalmennuksena. Voit valita sinulle sopivamman vaihtoehdon. Aika valmennukseen varataan neuvolakäynnillä tai voit varata sen sähköisesti Maisasta. Huomioithan, että etävalmennukseen osallistuminen puolison tai tukihenkilön kanssa onnistuu parhaiten yhteisesti yhden laitteen äärellä odottajan Maisa-tunnusten kautta. Valmennuksen kesto on noin kaksi tuntia. Valmennuksessa keskustellaan tulevasta elämänmuutoksesta, vanhemmuudesta ja läheissuhteista sekä vauvan ja vanhemman varhaisesta vuorovaikutuksesta sekä mitä ja miten vauva syö. Valmentajina toimivat neuvolan terveydenhoitaja ja perheohjaaja.

Health centres employ a wide range of health professionals. For non-urgent ailments and symptoms, you can be served by a doctor, nurse, physiotherapist or mental health and substance abuse specialist, among others. Non-urgent health matters include, for example, follow-up visits for a long-term illness, requests for test results, medical reports and prescription renewals. The service is available both online and by phone. When you contact the health centre, your situation will be assessed and you will receive guidance and advice. If needed, you will get an appointment with a health care professional. Together with a multidisciplinary team of professionals and you, we can prevent diseases and promote and support your health and wellbeing.

Conditions

You can contact us through the Klikkaa oire online symptom checker or Maisa. The service is intended for clients of the wellbeing services county’s health services. The urgency and nature of the service are based on a health care professional's assessment of the need for treatment. The guidance is always based on your eating habits, which will be assessed at the beginning of the appointment. If needed, your nutritional status or nutrient intake can be assessed even further. With personal and practical guidance, we support you in achieving your health-related goals and treating your condition. Your needs for nutritional therapy will be assessed by a doctor, nurse or other social and health care professional, who will then refer you to the therapy. Nutritional therapy can help you with, for example: - a long-term illness, such as coeliac disease, diabetes, kidney disease or inflammatory bowel disease - specific weight management problems - underweight or other form of malnutrition - difficult functional bowel disorders

The guidance is always based on your family's eating habits, which will be assessed at the beginning of the appointment. Personal and practical guidance supports your family's wellbeing and the adequate growth and development of your child. Occupational therapy can help you if you a struggling with everyday activities. Your ability to function may have declined because of an injury, illness or ageing. Occupational therapy looks for solutions to everyday challenges, for example, with getting dressed, cooking or running errands. An occupational therapist assesses your need for assistive equipment and home modifications. You can access occupational therapy through disability services, home care or other health care professionals. You can also apply the service yourself, or a loved one can do it for you. Occupational therapy is arranged as home visits and/or clinic appointments. The occupational therapist guides you and those around you in performing everyday tasks. The therapy period can be arranged individually, in groups or as remote appointments.

Description

Occupational therapy aims to improve the child's functional ability and performance in age-appropriate everyday activities. The reason for referral to occupational therapy is the child's difficulty in participating in age-appropriate activities in everyday life. Difficulties may appear, for example, in eating, dressing or playing, and in the background may appear poor coordination or other motor difficulties, learning difficulties, lack of concentration, challenges in fine motor skills or problems in play skills, interaction and relationships with friends. During guidance and assessment visits, the occupational therapist interviews the parents and the child and evaluates the child's skills and capabilities by observing the child in various functional situations and using standardized tests. If necessary, the occupational therapist cooperates with early childhood education or school staff. Based on the occupational therapy assessment, a follow-up plan is made together with the parents, which can include exercises and guidance for everyday life. Based on the assessment, individual or group rehabilitation can be recommended. The goals and methods of rehabilitation are chosen according to individual needs. In children's occupational therapy, functional capacity assessments and rehabilitation plans are made for children and young people with developmental disabilities. Description

Opioid substitution treatment is a rehabilitative or harm-reducing, long-term detoxification treatment where substitution dugs (buprenorphine or methadone) are used to treat withdrawal symptoms. In addition to medical treatment, you will receive psychosocial support (individual and group discussions) and support for life management. Before the substitution therapy starts, you will go through an assessment period where your goals are discussed and the suitability of the treatment is confirmed. Rehabilitative substitution treatment allows you to withdraw from substances and helps you build a substance-free life. Harm-reducing substitution treatment in turn aims to improve the quality of life and reduce the risks of substance use.

Conditions

The service is intended for opioid addicts. Before the substitution therapy starts, you will go through an assessment period. You can contact the substance abuse services to have your need for opioid substitution therapy assessed. At the end of the assessment period, a treatment plan is drawn up, which may include the start of opioid substitution therapy and/or another treatment option according to the treatment needs assessment. In the wellbeing services county of Vantaa and Kerava, adult oral health care services are provided based on the care needs of the individual. Regular oral and dental check-ups and good self-care prevent oral diseases. Oral health care follows a treatment plan that is made at the examination appointment. The plan outlines the schedule for the treatment and the approximate date of the next check-up and/or health examination. The services may include basic dental procedures and, if necessary, more demanding specialised treatment, according to the individual care plan. Oral health care is part of health services and works together with social services and specialised health care. Our oral health care professionals give you advice on oral health and guidance on self-care. Good and thorough self-care is important in maintaining oral health. Parents expecting a child Parents expecting their first child can participate in a free assessment of their oral health and care needs. The assessment is part of maternity and child health clinic services. During your visit, we will discuss your personal oral care needs and offer you advice accordingly. More frequent check-ups are also possible based on the needs of the child. The check-ups are carried out by oral hygienists and dentists. The role of oral health care is to promote the oral health of children and young people and to prevent and treat oral diseases. Your family can prevent your child's oral problems through good self-care. Our oral health care professionals will guide and support your child and family in self-care. Oral health care services for school-age children include oral health advice, oral health and dental check-ups as well as treatment according to the individual treatment plan. At each check-up, the child’s occlusion (bite) is assessed and, if necessary, they are referred to a specialist dentist for orthodontic screening. After basic education, schoolchildren are covered by student health care.

Conditions

The service is intended for schoolchildren living in the wellbeing services county of Vantaa and Kerava. Periodic oral health check-ups are carried out for all schoolchildren in grades 1, 5 and 8 in line with the regulation. In addition, the aim is to assess the care needs of pupils in grade 3 to monitor the development of the child's occlusion (bite) and to promote oral health. Based on the individual risk assessment, oral check-ups can be more frequent. Oral check-ups are usually arranged at a dental clinic near the school. Where possible, check-ups can also take place on school premises or in a mobile dental clinic during the school day. If the guardian is not present at the visit, the student will receive a take-home report on their oral and dental health. If the child has been in school since the beginning of the school year, the parent or guardian does not need to make an appointment for the periodic check-up. However, if the child has switched schools in the middle of the school year, the guardian is asked to book the appointment. All students studying in the Vantaa–Kerava region who are covered by student health care can book an oral health check-up at the wellbeing services county’s oral health care services once during their studies, regardless of their place of residence. Student health care covers students in preparatory education for an upper secondary qualification (TUVA), upper secondary education and basic education for immigrants. For more information, please contact your educational institution. Those living in the wellbeing services county will receive a letter from the oral health care services once they turn 17 to book the check-up appointment. You need to book the appointment yourself. You can prevent oral diseases through good oral hygiene. Oral health care services for students include oral health advice, oral health and dental check-ups as well as treatment according to the individual treatment plan. Please note that Finnish Student Health Service (FSHS) provides health care services for university and university of applied sciences students.

The services are available to all students at the Vantaa and Kerava wellbeing services county’s educational institutions who are covered by student health care. Description

At each oral health check-up, the child's occlusion (bite) is assessed and, if necessary, the child is referred to a specialist dentist for orthodontic screening. Orthodontic treatment requires that the malocclusion (bite defect) meets the national criteria for the treatment. If your bite defect does not meet the criteria, you can seek private orthodontic treatment at your own expense if you wish. We do not provide orthodontic treatment to alter the appearance of teeth but to fix functional issues. The treatment requires a long-term commitment, as it can last several years. The objectives of the treatment and the treatment plan will be presented to you before the treatment starts. Starting the treatment is voluntary.

Conditions

Orthodontic treatment requires that the malocclusion (bite defect) meets the national criteria for the treatment. At each oral health check-up, your child's occlusion (bite) is assessed and, if necessary, the child is referred to a specialist dentist for orthodontic screening. If you cancel your screening appointment, make sure to book a new one yourself. If you accept the treatment, you must follow the treatment schedule and instructions. You can influence the outcome of the treatment by using the appliances accordingly. The treatment may be stopped if you do not follow the instructions for using the appliances or if you have missed three or more appointments. The need for orthodontic treatment is primarily assessed during the statutory periodic check-ups. If necessary, you are referred to an orthodontic specialist for screening. Description

Other social care services are goal-oriented and planned support services that aim to improve your child’s wellbeing and support you as a parent. The goal is to promote your child's safe growth and development. The services include, for example, professional support person activities, professional weekend support activities, temporary house cleaning, provision of housing and support for young people’s independence, family and/or couples therapy, rehabilitative and therapeutic interventions in early family interaction, group services and neuropsychiatric training.

Conditions

The services are for client families of the wellbeing services county with children aged 0–17 where the child is a client of social care or child welfare services. Your child must have been assigned a personal social worker or a social worker responsible for the child's affairs in child welfare in accordance with the Social Welfare Act. You can apply for the services with an application made by a personal social worker/responsible social worker under the Social Welfare Act or the Child Welfare Act.

Parisuhde- ja eroneuvonnassa pääset keskustelemaan parisuhdetyön asiantuntijamme kanssa joko yksin tai yhdessä puolisosi kanssa. Soita lapsiperheiden neuvonnan ja ohjauksen numeroon 09-4191 5200 ma-pe klo 9-12 ja varaa aikasi parisuhde- ja eroneuvontaan. Voitte tulla tapaamiselle yksin tai yhdessä. Description

Perhehoitajien terveystarkastus on maksuton palvelu, jossa arvioidaan hyvinvointia, terveyttä ja jaksamista. Tarkastuksen tekee terveydenhoitaja, ja se voi sisältää laboratoriotutkimuksia sekä terveysneuvontaa. Tavoitteena on tukea perhehoitajan jaksamista ja ehkäistä sairauksia. Varaa aikasi hyvinvointi- ja terveystarkastukseen valitsemaltasi Vantaan ja Keravan hyvinvointialueen terveysasemalta puhelimitse tai sähköisesti. Tarkastukseen osallistuminen on vapaaehtoista, mutta suositeltavaa. Tarkastuksessa olisi hyvä käydä kahden vuoden välein. Tarkastus on maksuton. Hyvinvointi- ja terveystarkastuksessa terveysasemamme työntekijä tarkastaa yleisvointisi ja voit keskustella terveyteesi ja hyvinvointiisi liittyvistä kysymyksistä. Voit varata itsellesi ajan hyvinvointi- ja terveystarkastukseen terveysasemiltamme. Voit valita itse, millä terveysasemalla haluat asioida. Tarkastuksen tarkoitus: Hyvinvointi- ja terveystarkastuksen tarkoituksena on arvioida ja lisätä perhehoitajan hyvinvointia sekä ehkäistä sairauksia. Tarkastukseen osallistuminen on vapaaehtoista mutta suositeltavaa. Tarkastuksen sisältö: Tarkastuksessa käsiteltäviä asioita ovat muun muassa koettu terveydentila, mieliala, voimavarat ja jaksaminen, toimintakyvyn arviointi ja ylläpito, sekä terveyden edistäminen ja sairauksien ehkäisy. Tarkastus sisältää tarvittavat laboratoriotutkimukset. Tarkastuksen tekee terveydenhoitaja, ja tarvittaessa perhehoitaja ohjataan jatkotutkimuksiin, hoitoon tai kuntoutukseen oman asuinhyvinvointialueensa terveydenhuoltoon.

Toimintaohje päätoimisille perhehoitajille hyvinvointi- ja terveystarkastuksen varaamiseen: Alle 100 km säteellä asuvat perhehoitajat: Ajanvaraus: Voit varata ajan hyvinvointi- ja terveystarkastukseen valitsemaltasi Vantaan ja Keravan hyvinvointialueen terveysasemalta puhelimitse tai sähköisesti. Kanta-suostumus: Ennen ajanvarausta käy antamassa Kanta-suostumus Omakannassa. Tarkastukseen saapuminen: Saavu varaamanasi aikana terveystarkastukseen ja ota mukaan henkilötodistus sekä terveydentilaan liittyvät asiakirjat. Description

Personal assistance is a service where a person with a severe disability is allocated personal assistance hours. The aim of the services is to make it possible for you to live an independent life and participate in society on an equal footing. Personal assistance may focus, for example, on daily activities, work, studies, hobbies, social participation or maintaining social connections. The need for assistance is individual and its content depends on your life situation and need for help. As the client, you guide the assistant in their work. The decision on personal assistance is made based on a service needs assessment. The decision is either temporary or indefinite. It is temporary if your ability to function or circumstances can be expected to change in the next few years. The decision on personal assistance may also be changed if the legislation changes. As a client, you are obliged to report any changes in your ability to function and circumstances.

Conditions

You may receive personal assistance if you cannot carry out daily activities and manage at home without it; your need for assistance is high, ongoing or recurring at different times of the day or night. The person in need of personal assistance must have the resources to determine the content of the assistance and how it is provided. Personal assistance is mainly provided on weekdays between 8.00 and 18.00. For evenings and weekends, assistance hours are granted based on an actual assessed need when it is necessary for the client. Personal assistance is not granted when the need for assistance is mainly based on care, treatment, supervision and rehabilitation needs. You can contact the counselling service for people with disabilities either by phone, email, through the Maisa client portal or by sending an application online or in paper form.
