After-care supports young adults as they transition to independent living. It is also available to those who are moving home to live with their parents. In addition, after-care helps parents and others close to the young person. After-care can provide you with: - personal guidance and advice - help to find your own resources - help to plan for the future - help to find a home - support for living on your own - support for studies and job search - financial support and guidance After-care services are always based on an after-care plan, and participation is voluntary.

The services are available to you if you are 18–25-years-old and have been taken into care or placed in substitute care for at least six continuous months. Moreover, no more than five years have passed since you were a client of child welfare services. You are told about after-care towards the end of your placement or when you turn 18. Your social worker in child welfare will apply for the after-care services for you. If you have not been assigned an after-care worker or want to find out whether you are eligible for after-care, you can contact the after-care unit directly. We will find out whether you have the right for after-care and assign a designated worker to you.

Sometimes the resources and means of the family are not enough to ensure the growth and development of a child or young person. In such case, care and support can be provided outside the home. The child or young person can be placed either with a family or in a child welfare institution. A social worker makes the placement decision based on the Child Welfare Act. For the duration of the placement, your child is assigned a personal social worker of the municipality who is responsible for the child’s affairs. They draw up a client plan together with the child, the guardians or other family members and the substitute care place and follow its implementation. The child and the family receive support according to the plan. The social worker’s role is to monitor the child’s situation in the institution and to visit them regularly. The child's chances of returning home are assessed throughout the process. Child welfare institutional care is provided at a reception facility, children’s home, youth home, correctional school or another child welfare institution. There are three levels of care: Primary institutional care is for children with severe symptoms who need rehabilitation that can only be provided around the clock in institutional care without a guardian. The child may have severe behavioural issues, psychological symptoms or substance abuse problems or show antisocial behaviour. Specialised institutional care is for children with severe and/or dangerous behavioural issues who are a threat to themselves or others, have difficulties settling into care and need strong boundaries. The child may show, for example, severe psychological symptoms, heavy substance abuse or aggressive, antisocial or delinquent behaviour. Demanding institutional care is for children with severe mental health and substance abuse problems as well as delinquent and antisocial behaviour that require care so demanding that it cannot be adequately provided by the services available in the current service system. The wellbeing services county of Vantaa and Kerava has reception and assessment units as well as rehabilitation units. In addition, institutional care is provided on an outsourced basis as needed.

Child welfare is primarily based on open care support measures. If the child’s health or growth and development are seriously threatened by lack of care or other circumstances, we must resort to substitute care, which usually takes the form of family care. If family care is not sufficient or otherwise suitable, the child may be placed in institutional care. Placement in institutional care requires a client relationship with child welfare services and a placement decision made by the social worker of the municipality responsible for the child’s affairs. The need for institutional care placement is assessed on an individual basis, taking into account the child's needs and interests, in cooperation with the guardians, the child and the child welfare social worker. The placement decision is made by the social worker responsible for the child. You can contact the emergency child welfare service if you feel that the health and development of a child close to you are endangered by the child’s growth environment or their own behaviour.
