At-home hospital care offers you personalised and patient-centred hospital-level care at home, as an alternative to inpatient care. The services may include intravenous medication and hydration, blood transfusions, oxygen therapy, advanced wound care and pain relief, among other things. At-home hospital care can also be provided as outpatient clinic visits to the At-home Hospital. If necessary, you can be transferred to a hospital ward for treatment. If you have an incurable, terminal illness, we can help you stay at home for the rest of your life. At-home hospital services are provided around the clock, every day of the year. You will be cared for by a multidisciplinary team of doctors and nurses.
Условия
The services are intended for clients of the wellbeing services county’s health services who are over the age of 16. You must have a medical condition that requires hospital-level care but can be treated at home if you are well enough. You are expected to be able to cope with everyday activities independently or with the support of a relative, home care or nursing home. You must be substance-free for the duration of the treatment. You need a doctor’s referral to access at-home hospital care. If you wish, you can discuss the referral with your doctor. The At-home Hospital will assess whether the services are right for you based on the referral.
